CrossOver Healthcare Ministry Back-to-School Evening Physical and Vaccine Clinics
August 17, 2023 @ 5:30 pm – 8:30 pm
Community vaccine clinics are for non-CrossOver pediatric patients who are uninsured or carrying Medicaid. Preregistration is required by calling 804-655-2794, option 6. Bring photo ID, vaccine records, and school notice of missing vaccines (if available). Children must be accompanied by a parent/guardian.
